The online nominations process for ACM Senior Member/Distinguished Engineer, Scientist, or Member Nominations allows for a nomination to be saved and completed at a later point.
The following list provides a checklist for the information a nominator should
provide in order to complete the nomination.
Required information is indicated by (*).
Candidate Information:

- (*)Email, (*)Phone, Fax
- (*)Complete business mailing address
- (*)Complete home mailing address
- (*)ACM Member Number - or - ACM username
- (*)Accomplishments
- Summary of the major accomplishments of the candidate (4,000 characters maximum).
- (*)Education
- Name and location of educational institution, major, degree, and year of program completion. Order most recent first.
- Explanation of Education
- Nominator can explain specific aspects of the candidate's education.
- (*)Professional Experience
- Name of organization, position held, start- and end-year. Order most recent first.
- (*)Summary of Professional Experience (Applies to Senior Member only)
- Summarize candidate's professional experience (1,500 characters maximum).
